NW Natural Championships – April 26-27, 2013 in Boise, Idaho

	Online Application:- You will be asked to create a password. Using your e-mail address and this password, you can later make changes to your application, including changing weight classes or updating the Competitor Bio information. (Select “APPLY NOW” button below)

ENTRY FEE: Amateur Bodybuilding, Figure, Bikini and Men’s Physique

• $55 entry fee
• $25 crossover fee (to add a second class)
• $45 polygraph test fee
• $25 late fee for applications received on or after April 18th, 2013
  

Your entry Fee Includes PRE-JUDGING & NIGHT SHOW for the day of the event at which you are competing.

All  contestants will do their routine or walk at the evening show.
The top five amateur Bodybuilder, Pro Division, Figure, Bikini, and Men’s Physique  competitors in each class will then be brought out for awards.
All competitors MUST BE a minimum of 7 years drug free.

WAIVER:

- Competitors will be asked to agree to this waiver before submitting an application form.

By your accepting my application to the 2013 NORTHWEST NATURAL SPRING SHOW, along with my non-refundable entry fee, I hereby intend to be legally bound for myself, my heirs, executors and administrators, and waive and release any and all rights and claims for damages I may have against Northwest Natural Spring Show and/or Northwest Productions (Owner Allen Bowlden), the contest venue (Borah High School Auditorium), The National Gym Association Inc. (NGA), and any and all sponsors, agents, representatives and assigns for any and all injuries and/or losses suffered by me as a result of my participation and/or attendance and traveling to the 2013 NORTHWEST NATURAL SPRING SHOW, being held April 26th and 27th,, 2013. These damages include, but are not limited to, published photographs that I may find to be unattractive or editorial which I may construe as being misrepresentative.

I also grant permission to use photos, videos or any likeness of myself to promote any future contests, videos, magazines or any other media involved with future and present contests for the purpose of promotion and/or sales of these media, without any compensation to myself, as condition of my entering the event.

I also agree, by my own free will and without duress, to take a polygraph test and to submit a urine sample if I am asked to do so. The purpose of the tests is to identify any use of any anabolic steroids, growth hormone, prescription diuretics or substances banned by the National Gym Association by me within the last seven years. I also agree to the outcome of the tests’ results for both the polygraph and urinalysis tests. I realize I will be disqualified if I fail either test and in case of my disqualification, I will receive no refunds of fees.

I will abide by all NGA rules and show good sportsmanship. I understand that the promoter reserves the right to refuse or disqualify any competitor due to any behavior that he deems inappropriate. I understand that alcohol, tobacco, drugs and weapons are not allowed at the contest venue or on school property, and using or possessing questionable items will result in my immediate disqualification.

By the signing and submission of this form I am stating that I am at least 7 years Drug Free and will abide by the NGA drug test policies and procedures.
